Health Homes Serving Children

Overview

Our Health Homes Serving Children (HHSC) program is a specialized care management service tailored for children from birth to 21 years of age with behavioral or medical conditions. Our program is designed to provide personalized care for children and their families, with a primary focus on keeping the child at home and out of residential care. Services are community-based, meaning that they are offered within the child’s home or other comfortable, select community locations such as parks, schools, or libraries.

HHSC Services

Our HHSC Care Management Services offer specially trained Care Managers who specialize in working with children experiencing serious emotional and behavioral issues. Our professionals conduct regular home visits, collaborating with parents and community service providers. The services include crisis intervention and the arrangement of therapeutic, educational, recreational, respite, medical and family support services. 

Is this service right for a child in my life?

HHSC may be a right good fit for a child if they are under 21 years old and qualify in one of the following categories: Seriously Emotionally Disturbed, HIV, Complex Trauma, Sickle Cell Disease, or if they have two or more qualifying medical conditions.
