Health Home Serving Adults

Overview

Health Home Serving Adults (HHSA) is a care management service that supports the comprehensive needs of each individual so that they can remain healthy in their community while avoiding hospitalization. We assist individuals with coordination of care, collaboration with treatment providers and insurance plans, referral and linkage to community supports (e.g. psychiatric, medical, therapeutic, and social supports), as well as:

  • Information and help applying for entitlements (e.g. SSI/SSD, temporary assistance, food stamps),
  • Linkage to housing resources,
  • Medical appointment scheduling and appointment reminders, 
  • Nursing consultation, 
  • Obtaining transportation to and from appointments, 
  • Assistance with discharge planning if you’re in the hospital, and
  • Advocacy with service providers.

Is this service right for me?

Our HHSA care management program may be a good fit if you are aged 18 and older, have a chronic or ongoing health condition(s), and are insured by Medicaid, with limited availability for those with private insurance or no insurance.
