Home and Community Based Services (HCBS)

Overview

Our Home and Community Based Services (HCBS) are recovery and wellness services for individuals with mental health and/or substance use disorders. Our team provides support that can help an individual achieve the things they want such as a better social life, employment, or education.  

HARP-HCBS Services

Individuals can access HCBS services right in their homes or communities. Our services help people with: 

  • Independence
  • Managing Crises
  • Education and Employment
  • Peer and Family Supports

Is this service right for me?

Community Oriented Recovery and Empowerment (CORE) Services may be a good fit if you are eligible for the Health and Recovery Plan (HARP). HARP eligible individuals include those who are 21 years or older, are insured by a Medicaid managed care plan, and are enrolled in a Health Home and have a Care Manager.
