ACHIEVE – System of Care High Fidelity Wraparound

Overview

ACHIEVE – System of Care High Fidelity Wraparound is a care management program designed for Seriously Emotionally Disturbed (SED) children between 5 and 21 years old. Unlike traditional care management and health home programs, ACHIEVE follows the evidence-based high fidelity wraparound model that emphasizes regular child-family team meetings. During these child-family team meetings, our highly trained staff collaborates with the child, caregivers, and siblings to create a comprehensive care plan for all family members that addresses various aspects of life and social determinants of health as well as supports the development of problem-solving and coping skills.

  • 24-Hour Telephonic Crisis Intervention: Our Care Managers are experienced in working with children who have serious emotional and behavioral issues and provide 24-hour telephonic crisis intervention services to support your family during challenging moments.
  • Comprehensive Service Arrangement: From therapeutic and educational services to recreational, respite, and family support services, we arrange a comprehensive suite of resources to meet the diverse needs of all family members.

Is this service right for a child in my life?

ACHIEVE may be a good fit for a child if they are:

  • Between 5 and 21 years of age,
  • Enrolled in Medicaid,
  • Recommended to receive a high level of care management services based on their Child and Adolescent Needs and Strengths (CANS) assessment,
  • Multi-system involved, and
  • At risk of or returning home from out-of-home placement.
