Mobile Outreach Team (MOT)

Overview

In partnership with Rockland Psychiatric Center, our Mobile Outreach Team (MOT) supports individuals who are currently in an inpatient mental health setting but beginning their hospital discharge, to help them regain a sense of independence through re-establishing community connections and developing resources, skills and residential supports for living self-directed lives. MOT is personalized to the needs and goals of each individual—allowing for unique and customized support.

MOT Services

In general, MOT helps people live more independent and self-directed lives, assisting with: 

  • Developing daily living skills e.g. shopping, meal preparation, cleaning, personal hygiene, etc
  • Learning public transit or other transportation needs
  • Friendly companionship to reduce isolating tendencies
  • Promoting social connections through day programs and community events
  • Benefits management and other financial needs
  • Providing additional medical and counseling supports
  • Linking to extended services such as Care Management, Vocational Services or ongoing treatment programs

How to access care
