Assertive Community Treatment (ACT)

Overview

Our Assertive Community Treatment (ACT) team provides treatment and care management services for adults, aged 18 and older, who have found traditional clinic-based outpatient services insufficient for their level of needs. Our multidisciplinary team provides a blend of approaches that enable individuals to receive enhanced, intensive services while continuing to live in their communities.

Our team aims to address a variety of needs crucial to an individual’s well-being, such as:

  • Psychiatric and Nursing Care

Our Team

Our ACT program offers a team approach, which means that you will come to know a group of specialists who will work closely together and can visit you at your home and in the community. Our team includes a psychiatrist, nurses, social workers, licensed mental health counselors, a vocational specialist, a substance use disorder specialist, and a peer specialist. With your permission, we will work collaboratively with other service providers and your family members. 

Referrals

Referrals for ACT services begin with the SPOA application for both counties. Westchester is coordinated through the Westchester County Department of Community Mental Health (DCMH). To initiate a referral in Westchester, please contact DCMH at (914) 995-5245. Rockland is coordinated through Department of Mental Health (DMH). To initiate a referral in Rockland, please contact DMH at (845) 364-2399.

Once eligibility has been confirmed and approved, a Program Director from Greater Mental Health will contact you to discuss your respite care needs and coordinate accommodations.
