Referrals for ACT services begin with the SPOA application for both counties. Westchester is coordinated through the Westchester County Department of Community Mental Health (DCMH). To initiate a referral in Westchester, please contact DCMH at (914) 995-5245. Rockland is coordinated through Department of Mental Health (DMH). To initiate a referral in Rockland, please contact DMH at (845) 364-2399.
Once eligibility has been confirmed and approved, a Program Director from Greater Mental Health will contact you to discuss your respite care needs and coordinate accommodations.
