Critical Time Intervention (CTI) is an evidence-based model focused on reducing hospitalizations for those with severe mental health conditions. It is designed to support individuals as they transition out of hospital settings and back into their community. Our CTI team works closely with clients during this vulnerable period, helping them access the care, stability, and community supports they need to thrive.

We have assembled a multidisciplinary team to ensure clients receive comprehensive, well-coordinated support. Greater Mental Health’s CTI team includes Care Managers, Peer Care Managers, Mental Health Professionals (clinicians), a Medical Liaison, such as a Registered Nurse or Licensed Practical Nurse (LPN), and a Team Leader who is a Licensed Master Social Worker (LMSW).
This blend of clinical, medical, and peer perspectives provides clients with holistic, person-centered support.
CTI is designed specifically for individuals leaving inpatient or emergency psychiatric settings who may benefit from short‑term, intensive transitional support.
Eligibility criteria:
Through 2029, Greater Mental Health’s CTI program has no insurance barriers, we can serve individuals who are uninsured or under‑insured.
