It is designed to support individuals as they transition out of hospital settings and back into their community. Our CTI team works closely with clients during this vulnerable period, helping them access the care, stability, and community supports they need to thrive.\r\n\r\nSome key aspects of the program are:\r\n<ul>\r\n \t<li><strong>CTI is phase-based<\/strong>.\r\n<ul>\r\n \t<li>Participants begin with <strong>intensive support<\/strong> and gradually steps down to <strong>lower-intensity care<\/strong> as clients become more connected to long\u2011term providers.<\/li>\r\n<\/ul>\r\n<\/li>\r\n \t<li><strong>CTI utilizes a care coordination model. <\/strong>\r\n<ul>\r\n \t<li>Our team <strong>focuses on case management, linkage to resources, and practical support, <\/strong>and we work in collaboration with the participants\u2019 current treatment provider.<\/li>\r\n<\/ul>\r\n<\/li>\r\n \t<li><strong>Our CTI program is mobile and person-centered<\/strong>: Staff meet clients <strong>where they are, <\/strong>in the community, at home, or wherever support is most needed.<\/li>\r\n \t<li><strong>First in Westchester<\/strong>: We are the <strong>first CTI team in Westchester County<\/strong>, part of New York State\u2019s statewide CTI&nbsp;rollout.<\/li>\r\n<\/ul>","show_sidebar":true,"sidebar":{"label":"reach out today","image":"","name":"","title_role":"","text":"To learn more about Critical Time Intervention (CTI) please contact our Team Leader, Jennifer Aleman, LMSW, MS at 845-540-2747 or&nbsp;alemanj@greatermentalhealth.org","links":null}}},{"acf_fc_layout":"component_page_intro","_acfe_flexible_toggle":null,"component_page_intro":{"short_title":"Our Team","copy":"We have assembled a multidisciplinary team to ensure clients receive comprehensive, well-coordinated support.
